Integrated Housing and Recovery Team (IHART)

The Integrated Housing and Recovery Team (IHART) provides comprehensive case management for unhoused adults (18+) served by the Santa Cruz County Adult Mental Health system. IHART works in collaboration within the housing continuum to link clients to connector services that support access to long-term, permanent housing. Our team facilitates referrals to therapy, occupational therapy, psychiatry, and peer-support based on individual needs.
